Clinical partnership

Your care plan. A coordinated path home.

Referring a patient is more than sending an order. It means matching clinical needs with appropriate services, keeping the treating clinician involved, and helping the family understand what happens next. Allegiant HomeCare supports that conversation with a patient-centered approach that respects your clinical direction.

From inquiry to coordinated care

  1. 1

    Start a professional inquiry

Provide your practice contact details. We can discuss referral requirements without collecting patient information through this website.

  2. 2

    Confirm fit and eligibility

Discuss the requested disciplines, service location, coverage requirements, and current capacity before making any assumptions about acceptance.

  3. 3

    Arrange the clinical handoff

Use an agreed, verified secure channel for orders and necessary records. Clarify who will sign and oversee the plan of care.

  4. 4

    Coordinate the plan of care

If the referral is accepted, coordinate assessment, visit planning, family education, and communication with the treating team.

Prepare for the clinical handoff

  • Referral order and requested skilled services
  • Clinical history relevant to the home health need
  • Current medication list and allergy information
  • Qualifying face-to-face encounter documentation, when required
  • Ordering clinician and plan-of-care contact details
  • Coverage information, service address, and patient consent

Prepare these items for the verified clinical handoff—not something to send through this website. Documentation requirements depend on the payer and individual circumstances.

Common coordination questions

Does calling us place a referral?

No. A phone call starts a professional conversation. Patient acceptance, coverage, and a start-of-care date must be confirmed separately.

Which patients may qualify for Medicare home health?

Patients generally need a qualifying skilled need, a clinician-directed plan of care, and homebound status, along with other Medicare requirements. Homebound does not mean never leaving home. Eligibility is determined individually; custodial care alone is not covered.

How should we send orders or medical records?

Confirm the receiving team and an appropriate secure transmission channel before sharing patient information. Do not send patient records through this website form or ordinary email.
